Care coordination has become increasingly more difficult due to the burden that COVID-19 has put on our current healthcare system. Well-designed, targeted care coordination can improve health outcomes for everyone. 

Our team helps clients, and their families navigate healthcare systems like: Methodist, Memorial Hermann, Chi St. Luke's, M.D. Anderson, HCA and Kelsey Seybold as well as Community Hospitals and Urgent Care Centers.

Patients are often unclear about why they are being referred from primary care to a specialist, how to make appointments, and what to do after seeing a specialist. We make all primary, specialist and social service appointments. We provide detailed after-visit summaries with clear next steps.

Specialists do not consistently receive clear reasons for a patient referral or adequate information on tests that have already been done. Primary care physicians do not often receive information about what happened in a referral visit. We ensure that everyone that is a part of the care team has all pertinent information detailed in a comprehensive care plan. 

Information that is lost, uncommunicated or miscommunicated can cause irreparable damage; and in the most severe cases, even preventable deaths! All of our client's care notes are in stored in a single source Electronic Medical Record (EMR). All medications, allergies, lab results, imaging, provider notes, medical and social service appointments  can be accessed securely.
